Connect Polyform to Slack to get real-time notifications when someone submits a form.
#Setting Up
- Open your form's Integrations tab
- Click Connect Slack
- Authorize Polyform in your Slack workspace
- Select the channel to receive notifications
#Notification Format
Each notification includes:
- Form name with a link to view responses in Polyform
- All response answers (up to 10 per message)
- Image previews for image-choice questions
- File links for uploaded files
#Customization
You can toggle Include answers on or off. When off, you'll receive a notification that a response was submitted with a link to view it, but the answers won't be shown in the Slack message.
