Creating Forms

When you create a new form in Polyform, you'll choose one of three methods.

#Polly AI Pro

Let Polly generate an entire form from a text description:

1. Select Polly AI as your creation method
2. Describe the form you need (e.g., "customer satisfaction survey with 10 questions")
3. Polly will generate a complete form with relevant question types, labels, and settings
4. Customize the result in the Builder as needed

Polly generates 5–10 questions with a mix of question types appropriate to the topic.

#Templates

Start from a pre-built template:

1. Select Template as your creation method
2. Browse available templates
3. Choose one to use as a starting point
4. Customize it in the Builder

#Start from Scratch

Create a blank form and build it yourself:

1. Select Start from scratch
2. You'll land in the Builder with an empty form
3. Add pages and questions using the + button
