TL;DR: Connect Polyform to Slack and get notified in any channel when a form is submitted. You can include or exclude answer details in the notification. Set it up in the form's integration settings with one OAuth click.
#How it works
- Connect Slack from the integrations tab in your form settings
- Authorize — a standard Slack OAuth flow adds Polyform to your workspace
- Pick a channel — select which Slack channel should receive notifications
- Configure — choose whether to include answer details or just a submission alert
That's it. Every new submission sends a notification to your chosen channel within seconds.
#What's in the notification
By default, notifications include a summary of the submission. You can toggle whether to include the actual answers — useful for quick triage without opening the dashboard, but something you might turn off for forms with sensitive data.
#When to use Slack vs. Zapier
Both can send notifications to Slack, but they serve different purposes:
- Native Slack integration — zero-config notifications. Connect, pick a channel, done. Best for "I want to know when someone submits."
- Zapier → Slack — more flexibility. Custom message formatting, conditional triggers, routing to different channels based on answers. Best for complex workflows.
If all you need is a ping in Slack, the native integration is faster to set up and has no external dependencies.
